BLEEDING

Severe bleeding is a major threat to life and always needs prompt action by the First Aider.

  1. Apply direct pressure to the wound using your hand or the casualty's hand

  1. Elevate the limb: 5.11 cm
  2. Apply a thick dressing and a firm bandage.
  3. Check circulation in the end of the bandaged limb.
  4. Get medical help.

EMBEDDED OBJECTS

  1. Apply pressure with pads around the embedded object.
  2. Apply a firm bandage to hold the pads in place. Get medical help.

EYE INJURIES

LARGE EMBEDDED FOREIGN BODY OR LACERATION

  1. Assist the casualty into a comfortable position.
  2. Tell the casualty not to rub or move the eye.
  3. Cover both eyes with a light dressing bandage. Get medical help.

SMALL FOREIGN BODY IN THE EYE

  1. Assist the casualty into a sitting position, head back and leaning to one side.
  2. Gently separate the eyelids.
  3. Rinse the eye with clean water or a saline solution. 4. Get medical help. 5. Do not use any object to wipe the eye.

SPINAL INJURIES

UNCONSCIOUS CASUALTY

  1. Gently roll the casualty onto their side with minimal twisting and movement.
  2. Support the head and neck in alignment with the rest of the body.
  3. Maintain support of the head and neck with both of your hands placed on either side of the casualty's head until help arrives. 4. Get medical help.

IMPORTANT: The management principles of DRABC always take priority in the care and management of a suspected spinal injury.

CONSCIOUS CASUALTY

  1. Tell the casualty to remain still.
  2. Immobilize the casualty's head by placing hands on both sides.
  3. Loosen any constrictive clothing. 4. Calmly reassure the casualty. 5. Get medical help.

DO NOT move the casualty unless danger is life-threatening.

BITES & STINGS

  1. Reassure the casualty.
  2. Assist the casualty into a comfortable position; usually lying on the back.
  3. Apply a pressure immobilization bandage over the entire limb. d. Get medical help.

  2. Apply a cold compress (wrapped in a towel/cloth) to the bite area. 3. Get medical help.

BEE STING

  1. Rest and reassure the casualty.
  2. Scrape off the sting sideways with a fingernail or sharp edge.
  3. Apply a cold compress (wrapped in a towel/cloth) to the bite area. 4. Get medical help.

BURNS & SCALDS

The immediate aim of treatment is to cool the burned area.

  1. Cool the burn under cold running water for a minimum of 15 minutes.
  2. Remove items of clothing/jewellery likely to retain heat.
  3. Cover with a loose sterile/clean dressing, e.g., handkerchief, pillowcase. 4. Elevate burnt limbs. 5. Get medical help.

CHEST PAINS

  1. Assist the casualty into a comfortable position.

  3. Assist the casualty to take their prescribed medication for chest pains

Chest pains can conceal fatal conditions. There must be no delay in seeking medical help.

DRABC - Action Plan

DANGER. RESPONSE. AIRWAY. BREATHING. CIRCULATION.

1. DANGER

The first step in DRABC is to look for, assess and where possible remove any dangers that can injure:

  1. The first aider
  2. The bystanders 3. The casualty

These dangers may include fire, moving traffic, poisonous gases, electricity, sharp objects etc.

CALL FOR HELP

When calling or sending a bystander for help, the following details must be given to the emergency services.

  1. The exact location - house number, street name, nearest cross street and/or landmark, town and even state

2. What has happened

3. How many people are hurt

  1. If anyone is trapped

5. DO NOT hang up before the operator has finished. Let the operator hang up first.

  1. If sending a bystander for help, tell him to come back to me after you make the call. This confirms that the call has been made.

2. RESPONSE

The second step in DRABC is used to determine whether a casualty is conscious or unconscious.

The response is easily checked by gentle shake and shout. This is done by

  1. Gently shaking the casualty's shoulders
  2. Calling out in a loud voice, "Can you hear me? Open your eyes."
  3. The casualty's hands can also be squeezed to get a response.

IF THERE IS NO RESPONSE

Gently roll the casualty onto the side while supporting the head.

3. AIRWAY

The third step in DRABC is to check and ensure the casualty's airway is clear of obstructions.

1. Turn the casualty on the side 2. Turn face slightly down and tilt head back 3. Open mouth and look inside for any obstructions 4. Sweep out any obstructions with your fingers.

By tilting the head back, the tongue is also moved away from the back of the throat, which helps clear the airway.

If the casualty's airway is clear or has been cleared, proceed to the next step of DRABC: Check Breathing.

4. BREATHING

The fourth step in DRABC is to check whether the casualty is breathing by 'Look, Listen and Feel'.

  1. Look for movement of the casualty's chest and upper abdominal area. 2. Listen for air movement out of the casualty's mouth and nose.
  2. Feel for air movement against the side of your face.

IF BREATHING IS ABSENT

Immediately start EAR (Expired Air Resuscitation), usually known as mouth-to-mouth resuscitation.

Mouth-to-nose resuscitation may also be used.

  1. Roll casualty onto the back. 2. Tilt head backwards. 3. Seal casualty's mouth. 4. Give up to 5 full breaths in 10 seconds. NOTE: For the first aider's protection, the use of a shield device is recommended.

RECOVERY POSITION

When breathing begins, place the victim on their side, head back, jaw supported, face pointing slightly towards the ground.

5. CIRCULATION

The last step in DRABC is to check whether the casualty has a pulse. The best place to check for a pulse is on either side of the neck.

  1. Place two or three fingers over the casualty's Adam's Apple.
  2. Allow the fingers to slide off the Adam's Apple into the curve between the Adam's Apple and the neck muscle.

  1. Start CPR if the casualty has no pulse.

  1. Place heel of hand on lower half of the breastbone.
  2. Compress chest 4-5cm 15 times, then give 2 breaths.

3. Repeat 4 times per minute.
